It’s easy to get a person sick. True, illnesses are different: there are those that pass easily and imperceptibly, and there are those that do not allow you to do anything. In the latter case, people have to suspend their main activity. They interrupt their studies or stop working, take sick leave at work.

Employees through no fault of their own cannot carry out their work duties, therefore, they are entitled to material compensation from the employer. You can get it only if you have a sick leave (another name is a disability certificate).

In order to receive payment from the employer, it is necessary that the sick leave is correctly filled out. How to do this and how to work with sick leave as a whole is described further in the article. Also, an example of filling sick leave will be given below. This material will mention the minimum wage (minimum wage). Specifically, based on the calculation of the minimum wage when filling out a sick leave.

It should immediately be taken into account that the sick leave passes electronic processing at a certain moment of its bureaucratic path, that is, it is scanned on a computer. Accordingly, the correctness of filling out the document will depend on whether the citizen to whom it was issued will receive social benefits or not.

What are the rules for filling out sick leave currently?

  1. Data in the disability sheet can be entered using a special printing device or manually.
  2. When using the manual filling method, only black ink should be used.
  3. Symbols for any way of filling the sick-list should not go beyond the boundaries of the cells.

The sick leave is issued by the attending physician.

If the sick leave is lost before it is handed over to the employer, then the citizen has the right to receive a duplicate of the document, the employer has the right to correct the mistakes made when filling out the sheet without changing the original form (however, each correction must be certified on the back of the disability sheet).

What is a sick leave?

A disability sheet has a specific pattern. This is a sheet of blue-blue color (gradient) with a certain texture on the front side.

The sick leave contains the following blocks (below is an example of filling out a sick leave almost completely, except for the last block):

  1. A block that is filled by the attending physician.
  2. The block, the design of which is the responsibility of the employer.
  3. The block, which is also filled by the attending physician, but leaves it at the medical organization where the document was issued.

The columns for filling in the sick-list are white cells, each of which is intended for a single symbol (a space is also a symbol).

Places for stamps are provided on the sick leave so that they do not overlap the necessary information.

Rules for filling out a disability sheet by a medical professional

When filling out a disability sheet, the attending physician should:

  1. Indicate whether it is a sick leave certificate - a duplicate or the original.
  2. Briefly write the name of the medical organization.
  3. Enter the period for which the employee is relieved of his immediate duties.
  4. Write the date of issue of the document.
  5. Indicate your patient’s data.
  6. Enter the name of the organization where the patient works.
  7. Indicate OGRN and address.
  8. Enter your position.
  9. Indicate your data and put a signature.

After all the data is entered on the disability sheet, the document is transferred to the organization where the patient is employed, for further filling.

In order not to make a mistake when filling out a sick leave, the employer must take into account the following nuances of this document:

  • when specifying the name of the working organization, the abbreviated name of the company must be indicated; if it is not there, the name must be entered in full;
  • the employer must indicate whether the employee’s workplace in the company is the main one or if he works part-time; this is necessary so that a citizen receiving a sick leave certificate may require a duplicate document for two employers;
  • the employer must indicate in the document the number that the Social Insurance Fund assigned to the company during registration;
  • the employer must also indicate the branch number of the Social Insurance Fund; it usually consists of 4 digits, but if it is a regional branch, a five-digit code is indicated;
  • Be sure to enter the employee TIN;
  • the sick leave must contain information on the conditions of social payments to the employee, which is reflected in the format of codes on the back of the sheet;
  • in case of an employee receiving a work-related injury, the act N-1 is indicated (the date the act was issued), otherwise the column remains blank;
  • if the working agreement is canceled, then a line is filled out indicating the start date of work in the company, according to this document (agreement); the working relationship is terminated if the employee did not assume his immediate duties within the prescribed period after the hospital (although he is still supposed to pay the benefit);
  • the sick leave shall indicate the insurance period of the citizen, that is, the period from the moment of registration and the beginning of making contributions to the Social Insurance Fund;
  • non-insurance period is also indicated (for example, when a citizen was in military service);
  • fit the opening and closing time of the sick leave;
  • the average salary is calculated, which is calculated for the period of the last two years; if a citizen has worked in the organization for less than two years, then the sick leave from the minimum wage is completed;
  • the employee’s daily earnings are calculated on the disability sheet, which is calculated as the average salary divided by 730;
  • indicates the amount that the Social Insurance Fund is obliged to pay to the employee;
  • the final amount that must be paid to the employee for the period of incapacity for work is entered; it is calculated as the product of the average salary and the number of days during which the employee did not appear at the labor post (the interest accrued for seniority is also taken into account).

Also, the employer is required to include in the sick leave of the employee the data of the director of the company (the initials can be entered without dots), the chief accountant and their signature. If the position of the latter is not provided for in the state, then the data of the director of the company are duplicated.

It is important to recall that printing should not cross the fill fields so that the scanner can correctly display all the information in digital form.

How to fill out a disability sheet from the minimum wage? To answer this question, it is not necessary to give a special, based on the calculation of the minimum wage, sample of filling the sick-list. It is enough to say only that in the column “average earnings” you should indicate the amount equal to the minimum wage * 24.

Filling sick leave by the employer is carried out either by the director of the company or the chief accountant. One of them (who draws up the sheet) puts his signature on the form. He is obliged to check the correctness of filling out the document. Anyone who has certified the form with a signature is responsible.

Personnel department staff must also fill out some sick leave columns. In particular, they must indicate the name of the company, the length of service of the employee and all other data that must be entered according to the job description.

If the job description of employees does not spell out the obligation to fill out sick leave, then it is therefore unlawful to demand this from them.

Editing a document

It often happens that an error is made when filling out a document. It doesn't matter for what reason, whether it's carelessness or even intentional data corruption. The main thing is that the defect must necessarily be corrected so that the employee legally receives his payments, and that the person responsible for filling out the doctor or the representative of the employer has no problems.

If the attending physician made a mistake in the form when filling out the document, the paper must be replaced. If there is a mistake in filling out the sick leave by the employer, the algorithm for correcting it is completely different.

Firstly, the employer can correct a mistake in the document without replacing the form.

Secondly, each correction must comply with certain rules, namely:

  • the erroneous record must be crossed out along (the main thing is to do it carefully);
  • the correct record must be put on the back of the sheet, assured by its signature, seal and the inscription: “Corrected to believe.”

If you make a mistake and when correcting, the one on whom the sick leave is drawn up may have problems with receiving social benefits, therefore, the design of this document should be carefully considered.

Registration of a disability certificate in electronic format

Thanks to the federal law number 86 dated 05/01/2017, the sick-list can also be filled out in electronic format. Data using a special automated system, the operator of which is the Social Insurance Fund, fall into a common database. In it, interested parties can access information from sick leave certificates of the citizens they need (naturally, within the framework of the legislation of the Russian Federation).

Who can be the stakeholders? These include:

  • employers;
  • representatives of medical organizations;
  • employees of the Social Insurance Fund.

A sample of filling a hospital employee of a medical institution is given below:

  • on the top left, near the QR code, check the box next to the desired inscription (primary / duplicate);
  • below, where the name of the medical organization is written in brackets, the name of the medical institution is indicated; for example, clinic number 76 g of Volgograd;
  • in the column below it is necessary to indicate the address of the medical institution; for example, Soviet Volgograd 36;
  • below - the date of issue in the format "12.11.2017", the OGRN is put opposite the date, for example, 1028201839374;
  • then the full name of the person on whom the certificate of incapacity is drawn up is entered (the name is written opposite “F”, the name “I” is opposite, the name is opposite, “O” is opposite, and middle name is indicated); in the nominative case (Ivan Ivanov);
  • Next, the date of birth is indicated (12/29/1978), a checkmark is put in front of the desired gender, and the reason for disability is indicated using the desired code (for example, code is 09, and the remaining cells remain empty, but if necessary, they should also be filled);
  • in the column “place of work, name of organization” the name of the company is written, for example, LLC Viking;
  • it is indicated below how an employee works in this company: either this is his main place of work, or he works in it concurrently (a check mark is put in front of the necessary record);
  • if necessary, the age, family relationship code and name (separated by a space) of the relatives of the caregivers are indicated (for example, if the mother takes leave due to the illness of the children);
  • in the column “exemption from work” the relevant information on the terms of exemption is entered (12.11.2017 - from which date, 11/15/2017 - what date), position of the doctor (for example, pediatrician), full name of the doctor (galochkin nn) and his signature; if necessary, you can specify two or even three periods of exemption from labor duties;
  • at the end of the block filled by the attending physician, a date is placed opposite the inscription “start working with” and the signature of the employee of the medical institution.

Upper and lower case letters are not distinguished.

Employer Filling

An example of filling in sick leave by an employer looks like this:

  • in the column “place of work, name of organization” the same shall be indicated as in the same section in the block of an employee of a medical organization;
  • opposite the name of the company it is written on what basis the citizen works in it (main place of work / part-time);
  • The registration number (for example, 7710051333) and the reporting code (for example, 77101) are indicated below;
  • Further the employee TIN and his SNILS number are entered;
  • then the insurance period is indicated (for example, 04 years 07 months);
  • then the period for which the citizen needs to pay the benefit is indicated;
  • below, his average salary and average daily earnings (up to cents) fit in;
  • Further, the amount to be paid by the Social Insurance Fund shall be indicated, and on the contrary, the total amount that must be received by the citizen for whom the sick leave is issued;
  • at the end are the names and initials of the head and chief accountant of the company, as well as their signatures.

Of course, both the medical professional and the person responsible for filling out the employer’s block need to certify all the entries with the seals of the organizations.

The pattern of filling in sick leave by the employer largely repeats the pattern of filling in by the employee of a medical institution.
