- Passing all the procedures and laboratory examinations prescribed by the doctor (general analysis of biological fluids, feces on eggs, worms; fluorography response, electrocardiograms, other procedures).
- Additional consultations of narrow specialists, if the diagnosis is required (for women, the conclusion of a gynecologist is mandatory).
- Dermatologist - makes a mark on the card about the absence of head lice and scabies (everyone is required).
- A note by the epidemiologist about the lack of contact with infectious diseases over the past three weeks at the place of residence (everyone is required).
- A second visit to the therapist to get a completed sanatorium card form.

The certificate for the sanatorium must contain the following entries:
- Epidemiologist (not earlier than three days before departure to the sanatorium)
- Dermatologist
- Gynecologist (women)
- Blood, urine and stool responses
- Prophylactic Vaccine Statement
- Records of the therapist about the disease with the diagnosis (according to the ICD-10 code), the presence of contraindications to certain types of spa treatment, allergic reactions in the patient and the last treatment in the sanatorium, if it has been passed.
- Records of other specialists and the results of additional studies, if necessary.
It is also necessary to check whether the passport data and the number of the insurance policy are correctly filled in the appropriate columns of the spa card, the availability of the necessary seals and signatures of doctors.
The return ticket in the card F 072 / U-04 is filled in by the health workers of the sanatorium (dispensary), issued to the patient at the end of treatment, and then filed on an outpatient card for examination to the attending physician.
