A medical policy is a document that testifies to a citizen’s right to receive free help from medical workers to the extent determined by the compulsory medical insurance program . It allows you to seek help from doctors in any corner of the country, regardless of place of registration. The document must be issued free of charge. It is issued at work or at points of registration of policies. The document must be kept by the insured citizen.
A medical policy is issued for a certain period, moreover, one document is issued only to one person. It is not necessary in cases where urgent assistance is required. The document is presented only with a passport. If the policy is lost or its owner has changed his place of residence (last name), it must be exchanged or a duplicate order. Thanks to this decision, everyone can count on the qualified help of medical personnel, on the respect on his part, on the possibility of choosing a doctor depending on their preferences.
Reform of the healthcare sector is underway, so the old-style medical policy must be replaced. Now a single form document will be used. The exchange procedure is based on the application of a citizen who wants to receive a new document.
Replacing a medical policy at the request of not the insured person, but his representative is also possible. It is necessary to present
personal documents of this representative, as well as a power of attorney, which allows you to carry out actions on behalf of the person receiving the new document. The application must be filled out manually. The form is issued directly at the point at which the documents were submitted. You can submit them in person by contacting the insurance department, or send them over the Internet. In this case, you need to get a document confirming that the application has been accepted, as well as a temporary certificate that allows you to use the insurance system. It will be valid only 30 days from the date of application, however, during this period you should already be given a new medical policy.
The procedure for re-issuing a document or receiving a duplicate must be carried out in such cases:
- change of personal data (place of residence, last name);
- poor condition of the policy, in which it is impossible to read the data;
- loss of a document.
Within the set time, you will receive a new medical policy. You will be informed about the date of receipt by phone or email. Along with the new document, you should definitely get a memo stating your rights and obligations, as well as the duties of medical workers.
It should be noted that there is not much time left before the end of the policy replacement period. Old samples will cease to be valid in 2014. Do not put off the procedure in a long box! The influx of people at the exchange points will be very large!
